LIFE COACHING AGREEMENT

This Life Coaching Agreement (the "Agreement") is entered into by and between True Shalom Coaching (the "Coach") and the undersigned individual (the "Client"). Please read each section carefully before signing below.

1. SERVICES PROVIDED
The Coach agrees to provide Christian life coaching sessions as agreed upon in the chosen coaching package or session tier. Coaching is a collaborative process designed to assist the Client in clarifying goals, creating action plans, and fostering personal and spiritual growth.
2. NATURE OF THE RELATIONSHIP (DISCLAIMER)
  • Not Counseling or Therapy: The Client acknowledges that life coaching is not professional counseling, psychotherapy, mental health care, or medical therapy. The Coach will not diagnose or treat any mental health conditions or medical issues.
  • Referral: If mental health or medical needs arise, the Coach reserves the right to refer the Client to a qualified healthcare or mental health professional.
  • Responsibility: The Client retains full personal responsibility for their own decisions, actions, choices, and outcomes resulting from the coaching relationship.
3. SCHEDULE, FEES & PAYMENT
  • Discovery Consultation: Free of charge (1 session per client).
  • Single Sessions: $50 per hour, payable prior to or at the start of the session.
  • Packages (90-Day / 6-Month): Payment for packages is due in full prior to the first session, or per an agreed installment arrangement.
  • Cancellation Policy: The Client must provide at least 24 hours' notice to reschedule a session without penalty. Cancellations made with less than 24 hours' notice may be charged the full session rate.
4. CONFIDENTIALITY
The Coach respects the privacy of the Client and agrees not to disclose any personal information, conversation details, or records shared during coaching sessions, except where required by law (e.g., harm to self or others, child or elder abuse).
5. TERM & TERMINATION
Either party may terminate this Agreement at any time with written notice. In the event of early package termination by the Client, unused prepaid single sessions will be refunded on a pro-rated basis minus an administrative fee.
6. CLIENT ACKNOWLEDGMENT & SIGNATURE
